We cannot send them to the countries where EMS service is suspended. If certificates need to be sent to the country where EMS service is suspended, please contact us before applying for them.
※After completing the payment, the postage or the difference between the postage you have already paid and the fee of the alternative postal service will not be refunded due to the suspension of EMS services.
Keio University's privacy policy requires us to confirm that those making the request are the persons in question. Therefore, we require those making a request to present an official form of identification. A proxy requesting a certificate must present his/her official identification, a copy of the principal's official photo identification, and the Letter of Authorization signed by the principal.
Opening Hours
Monday through Friday,10:00 a.m. to 4:00 p.m. (The office is open as normal during the spring and summer breaks as well as during lunch time.)
Note: Certificates cannot be issued on Saturdays, Sundays, national holidays, school holiday (January 10), and during the university summer and winter break periods as the office is closed. Certificates also cannot be issued during system maintenance and when other university events are taking place. These will be announced on our website.

Acceptable Forms of Identification
Passport, both sides of residence card (alien registration certificate), Japanese Driver's license, certificate of driving record (issued on or after April 1, 2012), Japanese health insurance card(be sure to conceal the insured person's symbol and number as well as the insurer's number).
Certificate of residence, employee ID card, and student ID card from another university are not accepted.
If you live overseas, your passport is the only acceptable form of identification.

Those who graduated, completed, or withdrew from their program in or after March 1991 can visit the offices on the other campuses (Hiyoshi, Yagami, Shonan Fujisawa, Shinanomachi, and Shiba-Kyoritsu) to apply for and receive their certificates.

Download the following application form and fill it out. If you have trouble downloading the form, please prepare a document that clearly states the following.
(a) Your name in English (If you require Japanese certificates, we also need your name in kanji or katakana.)
(b) Date of birth
(c) Affiliation (faculty or graduate school, department, major, class)
(d) Student ID number (Leave blank if unknown.)
(e) Month and year of graduation, completion, or withdrawal
(f) Type of certificate (English or Japanese) and number of copies required
(g) Whether the certificates should be sealed in envelopes and how (If necessary, give detailed instructions, e.g., which certificates should be placed in which envelope, how many certificates should be placed in each envelope.)
Note: Be sure you check with the persons you need to submit the certificates to before applying. We will not be able to seal certificates in envelopes once they have been sent to you.

Procedures to Change Name
1. Certification of Individual Records or the Family Register, or Residence Certificate printed with the maiden name, issued within the last three months/ Japanese Driver's License printed with the maiden name
Write the faculty or graduate school you graduated from, your department, major, class, student ID number (if you remember), year of graduation, current name in English and in kanji or katakana, date of birth, current address, telephone number, and place of employment in the blank space of the certificate.
If you are applying online, please upload an image of your certificate.
2. Identity document with your current name
If you are applying in person, please bring the original with you.
If you are applying by mail, please send a copy of your identity document.

After you complete the procedures with the Office of Student Services, we will notify the Office of Alumni Affairs of the changes. If this is unnecessary, please let us know.
FAQ
What kind of information is on a Transcript of Academic Record?
A Transcript of Academic Record contains your name, date of birth, faculty/department/major, date of admission, date of graduation, degree received, courses taken, the academic year you earned credits (not your year level), number of credits, and grade.
Is it possible to get my GPA certified?
Is it possible to have my class ranking certified?
Keio University does not officially rank its students so this cannot be provided. "Class ranking is not available at Keio University" will be written on the Transcript of Academic Record.
Is it possible to have the Grading System certified?
The Grading System is indicated on the reverse side of the Transcript of Academic Record.
I need to send my certificates directly to the overseas graduate school I am applying for. Can Keio University directly send the certificates to the overseas institution?
Yes, it is possible as long as we are able to confirm that the certificates need to be sent directly from Keio University to the overseas institution. Please see here for the necessary documentation.
Can I receive certificates by e-mail or FAX?
Only original copies of the certificates will be issued. We will not send copies by e-mail or FAX.

Can I find out the date of admission to and graduation from the undergraduate program?
It differs by academic year. Please see below and obtain a Certificate of Graduation as necessary.
