Information (Update on Nov.19, 2021)

  • Notice on Winter Break Campus Closure.
  • On October 1, 2021, the domestic express mail fee will be revised. Please refer to here in detail.
  • On June 1, 2021, EMS service to U.S.A. will be resumed. Please refer to here in detail.
  • From April 1, 2021(Thu.), service hours at Mita campus is 10:00-16:00 on weekdays.

Information of International Mail Delivery

Please refer to Japan Post Website if you need to send certificates overseas.
We cannot send them to the countries where EMS service is suspended. If certificates need to be sent to the country where EMS service is suspended, please contact us before applying for them.

※After completing the payment, the postage or the difference between the postage you have already paid and the fee of the alternative postal service will not be refunded due to the suspension of EMS services.


Protection of Personal Information

Keio University's privacy policy requires us to confirm that those making the request are the persons in question. Therefore, we require those making a request to present an official form of identification. A proxy requesting a certificate must present his/her official identification, a copy of the principal's official photo identification, and the Letter of Authorization signed by the principal.

Monday through Friday, 8:45 a.m. to 4:45 p.m. (The office is open as normal during the spring and summer breaks as well as during lunch time.)
Opening Hours: 10:00 a.m. to 4:00 p.m. (From Apr. 1, 2021)

Note: Certificates cannot be issued on Saturdays, Sundays, national holidays, school holiday (January 10), and during the university summer and winter break periods as the office is closed. Certificates also cannot be issued during system maintenance and when other university events are taking place. These will be announced on our website.

Acceptable Forms of Identification

Passport, both sides of residence card (alien registration certificate), Japanese Driver's license, certificate of driving record (issued on or after April 1, 2012), Japanese health insurance card(be sure to conceal the insured person's symbol and number as well as the insurer's number).
Certificate of residence, employee ID card, and student ID card from another university are not accepted.
If you live overseas, your passport is the only acceptable form of identification.

Making a Request through the Online Application System

 *It is possible to make a request through the Online Application System, but certificates can only be issued after Jan. 6, 2022(Thu.).


EMS service to U.S.A. will be resumed on June 1, 2021
Japan Post Office will resume EMS service to U.S.A. on June 1, 2021 by introducing a "tentative extra charge for EMS" on items bound for Zone 2 (Oceania, North America, Central America, the Middle East and Europe). Please refer to Japan Post Website in detail.
Therefore, the EMS postage will be revised as followings on the online application system.

Oceania, North America, Central America and the Middle East: 2,051 yen→ 2,451 yen
Europe                         : 2,251 yen→ 2,501 yen
※Cost for a return-mail envelope is included.

Information of International Mail Delivery
Please refer to Japan Post Website if you need to send certificates overseas.
We cannot send them to the countries where EMS service is suspended. If certificates need to be sent to the country where EMS service is suspended, please contact us before applying for them.

Revision of Domestic Express Mail Fee
Japan Post Office will change some service on letters, postcards and yu-pack on October 1, 2021 and reduce the fee for domestic express mail by about 10% as shown below. Please refer to Japan Post Website (Japanese only) in detail.

Domestic Express Mail Fee: 290 yen→260 yen (on and after October 1, 2021)


  1. Applications and payments for certificates must be made by campus, degree program, or affiliation. If you require certificates from more than one campus, apply separately to each campus, specifying the number of certificates you require each time.
    ※Please refer to here about certificates for those who were enrolled in correspondence courses or as non-degree students
  2. Certificate fee, postage, and service fee will be charged every time you make an application through the Keio University Application System for Certificates.
  3. The maximum number of certificates that can be issued per application is 10.
  4. You will be required to upload a digital copy of your identity document. Please prepare a JPG or PDF file of the document before you proceed with the request. The file size must be 300 KB or less.
  5. For certificates which are not listed, write the name of the certificate and where it should be submitted to in detail in the "Other" field. (E.g., curator certificate, librarian certificate, request forms [e.g., LSAC: Transcript Request Form; WES: Academic Records Request Form; NY Bar: Form Law School Certificate; etc.])
  6. Forms such as request forms which are fixed by an organization can be uploaded as PDF files. However, if you need to submit the original document, please send it to us by post and let us know of this in the "Notes" field.
  7. Only original copies of the certificates will be issued. Please contact us in advance when you need to send the certificates to the countries where EMS service is suspended.
  8. The mailing address must be the address of the applicant, an overseas graduate school, or an overseas organization. You can have your certificates sent to a maximum of 2 addresses per application. However, you cannot specify 2 addresses in Japan.
  9. Please let us know in the "Notes" field if you require the EMS tracking numbers. We will send you the tracking numbers by e-mail after sending out the certificates.
  10. After completing payment, cancellations and changes to the number of copies required or destination (postage) cannot be made. Please also note that certificate fee, postage (including express mail charges), and service fee will not be refunded under any circumstance. If additional certificates are required, please re-apply. (Postage, service fee, etc., will apply separately.)

Those who graduated, completed, or withdrew from their program in or after March 1991 can visit the offices on the other campuses (Hiyoshi, Yagami, Shonan Fujisawa, Shinanomachi, and Shiba-Kyoritsu) to apply for and receive their certificates.


  • Fill out an application at Counter No. 1 (certification services) at the Office of Student Services, B1F, South School Building, Mita Campus. (Click here for campus map.)
  • You will need to bring a form of photo identification issued by an official body. (See the section on Acceptable Forms of Identification.)
  • If you wish to receive certificates that cannot be issued on the same day by post, please prepare a return-mail envelope in advance.
  • It is possible to ask a proxy to request a certificate on your behalf. In this case, the proxy will need to present his/her official photo identification, a copy of your official photo identification, and the Letter of Authorization signed by you when making the request.

Making a Request by Mail from within Japan

  • It is possible to ask a proxy to request a certificate on your behalf. In this case, the proxy will need to send a copy of his/her official photo identification, a copy of your official photo identification, and the Letter of Authorization signed by you when making the request. (See the section on Acceptable Forms of Identification.)
  • If you did not graduate from Mita Campus, please apply to the campus you graduated from.
  • Certificates will not be issued if the application and/or documents contain errors or are incomplete. If we cannot get in touch with you for three months from the day we receive the documents, all documents will be returned.
  • Only originals of the certificates will be issued. We will not send you a copy by fax or e-mail.
  • Be sure to send the following.


1. A copy of your official photo identification

See the section on Acceptable Forms of Identification.
The copy must be a clear, full size copy.



2. Application form

Download the following application form and fill it out. If you have trouble downloading the form, please prepare a document that clearly states the following.
(a) Your name in English (If you require Japanese certificates, we also need your name in kanji or katakana.)
(b) Date of birth
(c) Affiliation (faculty or graduate school, department, major, class)
(d) Student ID number (Leave blank if unknown.)
(e) Month and year of graduation, completion, or withdrawal
(f) Type of certificate (English or Japanese) and number of copies required
(g) Whether the certificates should be sealed in envelopes and how (If necessary, give detailed instructions, e.g., which certificates should be placed in which envelope, how many certificates should be placed in each envelope.)
Note: Be sure you check with the persons you need to submit the certificates to before applying. We will not be able to seal certificates in envelopes once they have been sent to you.
(h) Your contact details (home address, telephone number, and daytime contact details [mobile number, e-mail address, etc.])



3. Certificate fee

  • 500 yen per copy. ※On April 1, 2021(Thu.), the certificate issuance fee was revised to 500 yen per copy.
  • We only accept money orders (郵便定額小為替, yūbin teigaku kogawase issued at any post office in Japan) or cash (Japanese yen only).
  • Do not fill out the Recipient (指定受取人) and Address and Name (おところ・おなまえ) fields on the money order. We may not be able to process it for payment if you fill out these fields. Please also note that we will not be able to accept money orders that were not issued in the past six months.
  • If you wish to send cash, please send it by registered mail. Money should not be sent by ordinary mail as this may lead to problems such as damage to envelopes containing coins.
  • We will not accept stamps as a form of payment.


4. A return-mail envelope (We will not send the certificates to an "in care of" address.)

  • We will only send the certificates by post (standard mail). We will not send them by registered mail or courier.
  • All certificates are A4 in size.
  • Envelope sizes to send certificates ・Regular envelope ("nagagata 3" size [120 mm × 235 mm], standard size envelope): certificates folded twice ・Large envelope for A4 size documents ("kakugata 2" size [240 mm × 332 mm], nonstandard size envelope): certificates are not folded ・Large envelope for B5 size documents ("kakugata 3" size [216 mm × 277 mm]; nonstandard size envelope): certificates sealed in envelopes
  • English certificates may consist of two pages so postage may differ.
  • Estimate of postage (domestic) ・If you place the stamps in the envelope instead of affixing them to the envelopes, we will return the surplus.

Letter of Authorization (When Making a Request by Proxy)

  • The letter must clearly indicate the principal authorizing the proxy to make a request for and receive certificates on his/her behalf.
  • Click here for a sample Letter of Authorization. If you have trouble downloading the file, please prepare a document that clearly states the following.
    1. Date
    2. Name (and seal or signature), date of birth, address, daytime contact number, and e-mail address of the principal<
    3. Name, date of birth, address, daytime contact number, and e-mail address of the proxy
  • If the principal is living overseas,
    - a copy of his/her passport photo page must be submitted.
    - the principal's signature on the Letter of Authorization must be the same as that on his/her passport.

Certificates for those who were enrolled in correspondence courses or as non-degree students

The certificates for those who were enrolled in correspondence courses or as non-degree students cannot be issued at Office of Student Services on Mita Campus.

Procedures to Change Name

Please note that this procedure is separate from the procedure to change your name at the Office of Alumni Affairs.

1. Certification of Individual Records or the Family Register, or Residence Certificate printed with the maiden name, issued within the last three months/ Japanese Driver's License printed with the maiden name
Write the faculty or graduate school you graduated from, your department, major, class, student ID number (if you remember), year of graduation, current name in English and in kanji or katakana, date of birth, current address, telephone number, and place of employment in the blank space of the certificate.
If you are applying online, please upload an image of your certificate.


2. Identity document with your current name
If you are applying in person, please bring the original with you.
If you are applying by mail, please send a copy of your identity document.
If you are applying online, please upload an image of your identity document.


After you complete the procedures with the Office of Student Services, we will notify the Office of Alumni Affairs of the changes. If this is unnecessary, please let us know.

FAQ

What kind of information is on a Transcript of Academic Record?

A Transcript of Academic Record contains your name, date of birth, faculty/department/major, date of admission, date of graduation, degree received, courses taken, the academic year you earned credits (not your year level), number of credits, and grade.

Is it possible to get my GPA certified?

If the student entered the university on or after April 1, 2017, his/her grade point average (GPA) will be shown on the Transcript of Academic Record. The students who entered the university prior to April 1, 2017 will not have a GPA shown on their Transcript of Academic Record. If the student is a student at the Law School, his/her GPA will be shown, regardless of when he/she entered the university.

Is it possible to have my class ranking certified?

Keio University does not officially rank its students so this cannot be provided. "Class ranking is not available at Keio University" will be written on the Transcript of Academic Record.

Is it possible to have the Grading System certified?

The Grading System is indicated on the reverse side of the Transcript of Academic Record.

I need to send my certificates directly to the overseas graduate school I am applying for. Can Keio University directly send the certificates to the overseas institution?

Yes, it is possible as long as we are able to confirm that the certificates need to be sent directly from Keio University to the overseas institution. Please see here for the necessary documentation.

Can I receive certificates by e-mail or FAX?

Only original copies of the certificates will be issued. We will not send copies by e-mail or FAX.
※Please contact us in advance when you need to send the certificates to the countries where EMS service is suspended.

Can I find out the date of admission to and graduation from the undergraduate program?

It differs by academic year. Please see below and obtain a Certificate of Graduation as necessary.
