  • Certificates cannot be issued on Saturdays, Sundays, national holidays, during summer and winter short term program periods, and on January 10 (Anniversary of Yukichi Fukuzawa's Birthday) because the office is closed.

Notes when applying for certificates on campus.

  • Those who graduated, completed or withdrew from the university after March 1991 can request and receive certificates in-person on other campuses (Hiyoshi, Yagami, SFC, Shinanomachi, and Shiba-Kyoritsu).
    Please refer here for details.
  • Certificates for correspondence courses, non-degree students, and teacher training courses cannot be issued at the Mita Office of Student Services Please see here for more details.
  • Certificate requests are taken and certificates are issued at the number ① window (Keio alumni certificates) of the Office of Student Services, B1F, South Building, Mita Campus. (See: Campus Map)
  • Please bring proof of identification (original copy) for the graduate.
  • You do not need to bring a personal seal to apply for or pick up certificates.
  • If you wish to receive a certificate by mail that cannot be issued on the same day, please prepare a self-addressed, stamped envelope in advance.
  • It is also possible to designate a representative to complete the application on your behalf.
    *Please provide a letter of authorization from the applying graduate and the graduate's proof of identification (with photo) as well as proof of identification for the representative party.

  • Please send the following four items (1. through 4.) in a sealed envelope to 5.
  • You may not apply over the phone, via fax, or by email.
  • It is also possible to designate a representative to complete the application on your behalf.*Please provide a letter of authorization from the applying graduate and the graduate's proof of identification (with photo) as well as proof of identification(with photo) for the representative party.
  • For those who graduated from campuses other than Mita Campus, please apply to the campus of your graduation.
    *Check here for certificates related to correspondence courses, non-degree programs, and teacher training courses.
  • Please note that if any of the information or documents are incomplete, a certificate will not be issued until we can verify information. All documents will be returned if we cannot verify information for more than 3 months after documents are initially received.

1. Proof of identification (a copy) for the graduate

2. Application form

Certificate application form for those who graduated, completed or withdrew from program [←Download the form and fill it out].
If you are unable to download the form, please write down the following required pieces of information.

(1) Full name (prior to any name changes), furigana and romanization for name (required for English certificates)
(2) Date of birth
(3) Undergraduate faculty or graduate school, department, and major
(4) Student ID number (if you remember)
(5) Year and month of graduation, completion of program, or withdrawal from the university
(6) Type of certificate (in Japanese or English), number of copies
(7) Whether or not envelope should be securely sealed / how the envelope should be prepared (e.g. details like which and how many certificates should be placed in one envelope) Securely sealed certificates will be placed in official Keio University envelopes, stamped, and sealed in a fashion that prevents tampering. Please check with the certificate recipient prior to making a request for instructions. Certificates we have already handed out cannot be securely sealed later.
(8) Contact information (home address, phone number, daytime contact information such as cell phone number or email address)


3. Fees for issuing certificates

500 yen per certificate. Prepare a fixed-amount postal money order (郵便定額小為替証書; yubin-teigaku-kogawase-shōsho) or cash (in Japanese yen)


  • Fixed-amount postal money orders (郵便定額小為替証書; yubin-teigaku-kogawase-shōsho) can be purchased at post offices.
  • Please leave the "Designated Payee" and "Address and Name" fields blank. If you don't leave the fields blank, Keio may not be able to process your payment.
    Fixed-amount postal money orders are valid until 6 months after issuance.
  • Cash should be sent by registered mail for sending cash. Sending by regular mail is strictly prohibited. Problems can occur, such as damage to envelopes due to the weight of coins.
  • Stamps will not be accepted.


4. Return envelopes: stamped, self-addressed, stamped envelopes with the applicant's name and address (no "care of" forwarding allowed)

    • We do not accept return methods other than by post (registered mail, courier service, etc.).
    • Certificates are A4 size.
    • Return envelope sizes:・If you wish for the certificate to folded in thirds, please prepare an envelope in the nagagata san-gō size (長形3号; 12 x 23.5 cm: letter size envelope) ・If you wish for the certificate to NOT be folded, please prepare an envelope in the kakugata ni-gō size (角形2号; 4 x 33.2 cm: large envelope)・If you wish for the envelope to be securely sealed, please prepare an envelope in the kakugata-san-gō size (角形3号; 21.6 x 27.7 cm: large envelope)
    • English-language certificates may become two pages, changing the price.

Certificates for correspondence courses, non-degree students, and teacher training courses

The Mita Office of Student Services cannot issue certificates for correspondence courses, non-degree students, and teacher training courses.

  • Only the Mita Campus Office of Correspondence Courses can issue certificates for correspondence courses. For more details, refer to the Correspondence Courses website.
  • For certificates for formerly enrolled non-degree students (For credit, non-credit, and research students), please consult the Academic Affairs Office for your former main undergraduate faculty/graduate school. For more details, refer to the non-degree students website.
  • For a "Certificate of Academic Proficiency" or other certificates, please refer to and apply via the Teacher Training Center website - Apply for certificates. *This is not a Transcript of Academic Record*

Certificates for 2nd year in-school transfer students

  • The path from enrollment to graduation is as follows:
    Enrollment (before transfer) Enrollment in 1st year of undergraduate studies → Enrollment (before transfer) Withdraw at the end of the 1st year of academic studies → Graduation (after transfer) Enrollment in 2nd year of academic studies → Graduation (after transfer) from undergraduate program.
  • Certificates available for issue:
    Enrollment (before transfer) Undergraduate - Certificate of Withdrawal / Transcript of Academic Record
    Graduation (after transfer) Undergraduate - Certificate of Graduation / Transcript of Academic Record
    Certificates for the faculty you initially enrolled in and the faculty you graduated from will be different. You cannot receive certificates that list both.
  • The Transcript of Academic Record for your faculty at graduation will not contain grades from your faculty of initial enrollment. However, if credits for courses taken at the undergraduate faculty of initial enrollment were recognized by your faculty at graduation, they will appear on the Transcript of Academic Record for your graduating faculty (the course titles, grades, and years may be different)
  • The date of admission with your faculty at graduation will reflect the date you transferred, and will be listed on certificates as the second year of enrollment.

Procedures for changing one's name or surname, or reverting to one's previous name

Name change procedures must be handled separately from that of the Office of Alumni Affairs.

Necessary documentation for changing your name (any of the following)

  • Certification of personal information issued within the last three months (copy of family register)
  • Certificate of residence listing both your current and previous names issued within the last three months
  • Driver's license listing both your current and previous names

The process for changing one's name or surname or reverting to one's previous name will differ depending on which way you make your request to issue a certificate. Please prepare your documents and check the application procedures on the following page.


Have you received your diploma yet?

If you were unable to attend your undergraduate or graduate commencement ceremony for unavoidable reasons and thus have yet to receive your diploma (Certificate of Graduation/Completion), you may either pick it up directly at the Office of Student Services desk at Mita campus or have it mailed to an address within Japan. We ask that you follow up on this matter as quickly as possible.

Diplomas can be picked up at the office or mailed to a proxy in Japan. If you choose to have someone receive your diploma on your behalf, they must present a letter of authorization and personal identification in addition to the documents listed below.(Example of a letter of authorization)
Proxies are requested to confirm in advance the graduate's undergraduate faculty/graduate school, department, major, student ID number, and the month/year they graduated from or completed their degree.

1. Picking up a diploma in person

The diploma will be given to you at the Mita Campus Office of Student Services at the #1 counter. Please bring proof of identification.

2. Receiving a diploma in the mail

Diplomas will be mailed to domestic residents by registered mail. Please send the following three items:
After Keio receives these documents, the diploma will be mailed within one week.
・Proof of identification(photocopy)
A request form for sending a diploma (←this form can be downloaded)

If you are unable to download the form, please write down the following required pieces of information.
Full name (maiden name), student ID number (if you remember it), undergraduate faculty or graduate school/department/major/group, month & year of graduation/degree completion, contact details during business hours and your email address.

・A Kakugata Size 2 return envelope size (332x240 mm or an envelope that will easily fit an A4 piece of paper)
Write your return address and name on the envelope and attach a 740 yen stamp for postage.

FAQ

If I apply online(received by post), how long will it take to receive my certificates?

Generally, applications will be processed the business day after the online procedures are completed. For domestic shipping, if there are no issues with documents and information provided, we will issue and ship certificates within 2 business days from the day your application is processed. For international shipping, we will send certificates via EMS 2-5 days after your application is processed. We cannot comment on how many days the shipping process itself will take. We ask for your understanding in this matter.

If I apply by mail, how long will it take to receive my certificates?

Generally, if there are no issues with the documents and information provided, same-day certificates will be issued on the day your application arrives in the mail and shipped out in the evening. To expedite the process, please prepare an express mail return envelope. We cannot comment on how many days the shipping process itself will take. We ask for your understanding in this matter.

I need to issue a sealed certificate. How do I apply?

  • Online requests (certificates received by post)
  • Requesting certificates in person at the office on the campus
  • Requesting certificates by mail (certificates received by post)

*Certificates printed at convenience stores cannot be "securely sealed" (sealed in an envelope and delivered).
*The issued certificate cannot be sealed. The sealed certificate need to be asked when it is applied.

Do I need my personal seal (inkan) when I apply for and pick up certificates in-person?

Not required if you are the graduate. If you are applying by proxy, the proxy is required to bring a letter of authorization stamped with the graduate's personal seal. The proxy's personal seal is not required.

How long does it take to receive same-day certificates when applying in-person?

Other than the certificates listed below, all certificates can be issued immediately.

Undergraduate program graduates from March 1948 - March 1977: Certificate of Graduation and Transcript of Academic Record (Japanese and English)
Master's program graduates from March from 1953 - March 1990: Certificate of Completion and Transcript of Academic Record (Japanese and English)
Those who withdrew upon completion of course requirements in March 1990 and before: Notification of Withdrawal upon Completion of Course Requirements and Transcript of Academic Record (Japanese and English)
Curator Certificate (Japanese and English)
Librarian Certificate (Japanese and English)

These certificates take around an hour to be issued. English certificates may take longer. For details, please contact us prior to applying.

Can I issue certificates from an on-campus certificate issuing machine?

You may not. Certificate issuing machines may only be used by current Keio students.

Can I use an employee ID, student ID card from another university, or a foreign driver's license as proof of identification?

You may not. Please prepare an approved form of proof of identification.

What information is listed on a Transcript of Academic Record?

Your full name, date of birth, undergraduate faculty/department/major or graduate school/major, date of admission, date of graduation/program completion, degree received, courses taken, academic years, and grades. Only the academic year in which a credit was earned will be listed, not your year level at the time it was earned.

I have to send request forms and certificates to institutions outside of Japan (WES, LSAC, NY Bar, NIES etc.) directly from the university I went to. Can you mail them directly?

When applying to graduate schools outside of Japan, certificates must be sent directly from universities to the destination. Can you ship them directly?

After applying online to send a certificate outside of Japan, I found out that the destination is in a country where EMS postal service has been suspended. If I cancel my order or request an alternative means of mailing, can I get my postage (or the difference between the two alternatives) back?

Once the payment procedure has been completed, the fee cannot be refunded for any reason. If you have any questions, please contact us before completing the payment process.

Can you certify my GPA?

  • If your date of admission is on or after April 1 of 2017, your GPA will appear on your Transcript of Academic Record. GPA will not be listed if your date of admission is March 31 of 2017 or before. For graduates of the Law School, your GPA will be listed on your Transcript of Academic Record regardless of your date of admission.

Can you certify my class ranking?

Keio University does not calculate rankings, so there are no official rank to report. English Transcripts of Academic Record will display "Rank in Class is not available at Keio University."

Can you certify the grading system criteria?

Grading system specifics are listed on Transcripts of Academic Record.

Can you issue a "Certificate of Completion/Certificate of Coursework Completion" for the certified public psychologist examination?

Can you certify my license to become a social welfare officer (社会福祉主事任用資格)?

We ask that you have the recipient of your Transcript of Academic Record check the necessary areas.

How long do surname change procedures take to process?

Procedures can be completed immediately.

I have changed my surname. Can I still receive certificates with my surname while I was enrolled at Keio University?

Yes. We ask that you prepare proof of identification with your current name. Please inform the staff at the counter that you do not require your certificate to reflect your present surname.

I need a Certificate of Enrollment for pension procedures. Can I have one issued?

Certificates of Graduation list the date of admission and graduation, which proves your enrollment for the period you were a student. Please request a Certificate of Graduation.

I want to know the specific date of enrollment, graduation, or program completion to put on a resume.

Those dates vary by academic year. See below. Please request a Certificate of Completion if necessary.

Can you confirm the enrollment of a deceased person?

We can confirm their enrollment via issuance of a certificate for graduation/degree completion/withdrawal from university. For those seeking such certificates, please contact us via email or by phone, and we will offer an explanation of the required documents. Please note that we do not accept requests in the following situations:

・To those who are not blood relatives
・When the reasons for making the request are unclear
・For the purpose of confirming anything other than enrollment
