Certificate issuance fee
500 yen per copy (for both Japanese and English certificates)
Current students who graduated or completed a degree at a Keio faculty or graduate school
Current students who previously graduated from an undergraduate degree program or completed a master's degree program at Keio University can issue certification for their previous enrolment (graduated/completed an undergraduate/master's degree program) using their current student ID cards. Note, however, that it may not be possible to issue certificates depending on the undergraduate or master's degree program or the academic year of graduation or completion of degree.

Information of International Mail Delivery
Please refer to Japan Post Website if you need to send certificates overseas.
We cannot send them to the countries where EMS service is suspended (e.g. U.S.A). If certificates need to be sent to the country where EMS service is suspended, please contact the office of campus you belonged to before applying for them.
※After completing the payment, the postage or the difference between the postage you have already paid and the fee of the alternative postal service will not be refunded due to the suspension of EMS services.
All procedures involved with certificate applications can be completed online.
Enrolled Keio students, correspondence course students, non-degree students (incl. non-degree students for credit/non-degree students (non-credit)/research students) cannot apply for certification of Keio affiliation using this system.

Application for certificates by post and at service desk on campus of graduation, completion, or withdrawal
Certificates are issued at Office of Student Services on each campus.
Application for certifications at other campuses than those of graduation, completion, or withdrawal
To prevent the spread of COVID-19, please request certificates through the online application system or mail.
Please confirm the following precautions when coming on campus.
Eligibility is for those who graduated, completed a degree program, or withdrew from the university from March 1991 onward.
However, please note that there will be variations by date of graduation, program completion, or withdrawal from university for the applicable courses at the School of Medicine, Graduate School of Medicine, Graduate School of Business Administration, Faculty of Pharmacy, and Graduate School of Pharmaceutical Sciences.
Postal applications and those for special certificates are only accepted at the campus from which persons graduated, completed a degree program, or withdraw from the university regardless of the academic year of graduation.
Same-day issuance of certificates is available from the service desks during service times at respective campuses.

Please only visit us after contacting us in advance regarding any points about which you remain unclear.
(Note 1) Proof of identification
The following are acceptable as proof of identification:Driver's license (Japanese), Certificate of Driving History (issued on or after April 1, 2012), passport, Health insurance card (be sure to conceal the insured person's symbol and number as well as the insurer's number), Residence Card.
Certificates of Residence, employee IDs, or student ID cards for other universities may not be presented as proof of identification. If none of these are available to you, please contact us in advance.
(Note 2) Letter of Authorization
There is no prescribed format for Letters of Authorization. Please refer to the below and draft the letter so the intentions of the student are clearly indicated.

For the attention of people who have collected certificates
Procedures for change of name or surname, resumption of previous name
Please submit the below documents. Certificates will be issued in the current name of the student after taking receipt of the documents as soon as the procedures are complete.
Note:There are different procedures for the Office of Alumni Affairs for changes to names.
We will report to the Office of Alumni Affairs following completion of procedures at the Office of Student Services. Please contact us if there is no need to modify the information registered with the Office of Alumni Affairs.
Please note that as it is not necessary to submit documents such as a copy of the family register for the name change procedures at the Office of Alumni Affairs, these do not constitute the procedures to change names on certificates.
Service hours and contact information
Certificates cannot be issued on Saturdays, Sundays, national holidays, school holiday (January 10), and during the university summer and winter break periods as the office is closed.
