【SFC】Certificates for Alumni

To prevent the spread of COVID-19, please request your certificates through the online application system or by mail if at all possible.

Precautions when coming on campus

  • When entering the campus, a body temperature check will be conducted, and persons with a fever of 37.5 degrees or higher will not be allowed to enter.
  • It is also mandatory to wear a mask.
  • Please return home immediately if you are feeling unwell or exhibiting symptoms of a cold, etc.

Certificates are issued only when all necessary information and documents have been submitted and verified. The office does not accept any changes in the content of the request that has already been submitted. Alumni must decide the number of copies and category of certificate that they wish to request before submitting required documents. Issued certificates are not refundable.


Because of the privacy policy, the University needs to confirm that the request is made by the alumnus/alumna in question. Therefore, the University requires an alumnus/alumna to present his/her official identification. A proxy requesting a certificate must present his/her official identification, a copy of an official identification of the alumnus/alumna and a 'Letter of Authorization' signed by the alumnus/alumna. A proxy must be someone in Japan who can request and receive certificates on behalf of the alumnus/alumna.

Monday through Friday, 9:15 a.m. - 4:50 p.m.

Certificates cannot be issued on Saturdays, Sundays, national holidays, school holidays (January 10 ) and during office closing periods in summer and winter breaks.

Information Required for Issuance

All alumni requesting a certificate are required to provide the following information to the Academic Affairs Office:




  • Student ID number for each undergraduate and graduate program (if you do not remember, leave it blank.)
  • Name in Chinese characters or katakana (and Roman alphabet if an English certificate is required)
  • Date of birth (and gender if an English certificate is required)
  • Month and year of graduation/completion/withdrawal
  • Name and language of certificate (English or Japanese)
  • Number of copies
  • Request to seal certificates (if necessary, provide concrete instructions.)
  • Present mailing address, email address, daytime telephone and cell phone numbers


Request in Person or through a Proxy at the Academic Affairs Office Counter

Request by Alumni

All alumni must present an official identification at the Academic Affairs Office counter to obtain certificates. The University accepts driver's license, passport, health insurance card, the Certificate of Alien Registration, and Basic Resident Register card (with a photo). Resident cards, employee ID cards and student ID cards of other universities are not accepted. After receiving a request form, all alumni should fill out the form and submit it to the office. If alumni wish to receive their certificates by mail, they must also submit a self-addressed stamped envelope with stamps.



Request through a Proxy

To obtain a request form, the proxy must present an official identification, a copy of an official identification of the alumnus/alumna and a 'Letter of Authorization' signed by the alumnus/alumna at the Academic Affairs Office counter to certificates. After receiving a request form, the proxy should then fill it out and submit it to the office.
The format of the 'Letter of Authorization' is not specified, but it must clearly state the alumnus's/alumna's intention of authorizing the task to the proxy.

Notes on applying for certificates

  1. Applications and payments for certificates must be made by campus, degree program, or affiliation. If you require certificates from more than one campus, apply separately to each campus, specifying the number of certificates you require each time.
  2. Certificate fee, postage, and service fee will be charged every time you make an application through the Keio University Application System for Certificates.
  3. The maximum number of certificates that can be issued per application is 10.
  4. You will be required to upload a digital copy of your identity document. Please prepare a JPG or PDF file of the document before you proceed with the request. The file size must be 300 KB or less.
  5. For certificates which are not listed, write the name of the certificate and where it should be submitted to in detail in the "Other" field. (E.g., request forms [e.g., WES: Academic Records Request Form; etc.])
  6. Forms such as request forms which are fixed by an organization can be uploaded as PDF files. However, if you need to submit the original document, please send it to us by post and let us know of this in the "Notes" field.
  7. Only original copies of the certificates will be issued. We will not send copies by fax or e-mail.
  8. The mailing address must be the address of the applicant, an overseas graduate school, or an overseas organization. You can have your certificates sent to a maximum of 2 addresses per application. However, you cannot specify 2 addresses in Japan.
  9. After completing payment, cancellations and changes to the number of copies required or destination (postage) cannot be made. Please also note that certificate fee, postage (including express mail charges), and service fee will not be refunded under any circumstance. If additional certificates are required, please re-apply. (Postage, service fee, etc., will apply separately.)
  10. Those requesting AUD Shutoku Shomeisho or certificates from the teacher training course should contact the Academic Affairs Office in advance.

Request via Mail from within Japan

Note: Students cannot request for certificates via mail.

Any requests made by telephone, fax, or e-mail will not be accepted.

All alumni or their authorized proxy must send the following documents to the Academic Affairs Office:



1. A Copy of Alumni's Official Identification

The University accepts only driver's license, passport, health insurance card, the Certificate of Alien Registration (copy of both sides) and Basic Resident Register card (with a photo). Resident cards, employee ID cards and student ID cards of other universities will not be accepted.



2. Self-addressed Stamped Envelope

  • A return-mail envelope must have the address of the alumnus/alumna or the proxy. The university does not send certificates to alumni in care of a third party.
  • All certificates are issued in A-4 size.
  • A return-mail envelope must be 240×190 mm (角5 Catalog Style) or comparable size when requesting sealed certificates or AUD Shutoku Shomeisho (AUD取得証明書).


3. Issuance Fee

All alumni or their proxies should enclose a money order (郵便定額小為替: yubin teigaku kogawase issued at any post office in Japan. Yubin teigaku kogawase is only accepted in this case) as an issuance fee, or send cash by registered mail. The name of the recipient or yubin teigaku kogawase should be Keio University Shonan Fujisawa Office.

Amount of Time Required for Issuance

The following table shows the amount of time required for the issuance of Japanese and English certificates when requested in person at the Academic Affairs Office counter:

Certificates cannot be issued on the day requested during the system maintenance period. The system maintenance schedule will be updated on this page once it is fixed. For more details, please contact the Academic Affairs Office.

Official Identification

The University accepts only driver's license, passport, health insurance card, the Certificate of Alien Registration (copy of both sides) and Basic Resident Register card (with a photo). Resident cards, employee ID cards and student ID cards of other universities will not be accepted.
*Only passport is accepted if Alumni's living overseas.

Letter of Authorization

The format of the 'Letter of Authorization' is not specified, but it must clearly state the alumnus's/alumna's intention of authorizing the task to the proxy.
