Change Name/Address/Guarantor
Please promptly carry out the required procedures if your address (or your guarantor's address), guarantor, or name has changed. If you do not carry out these changes, it could prevent you from receiving important messages from the university regarding course registrations and other matters.
Change of Address(Students or Guarantor)
How to Apply
Log in to K-Support, go to the left menu > "Applications" (on a smartphone, go to the upper right menu button > "Applications"), access "Confirmation/Change of Address (for Students/Guarantor)" and apply for the change.
Notes
- It will take about one day for the change of address to be approved (except on Saturdays, Sundays, national holidays, and other university holidays). When the change of address is approved, you will receive an e-mail to your keio.jp e-mail address to notify you of the approval. (However, if the only change items are your phone number and/or your e-mail address, no notification e-mail will be sent.)
- Due to the large number of applications received during the new semester, approval may take some time.
- Please note that the new address will not be reflected on a student commuter certificate that can be applied for and issued through the "Certificate Issuance Service" until 7:00 a.m. on the day after the change of address is approved.
- If you wish to apply for a student commuter certificate in order to purchase a student commuter pass, please apply for it from the "Certificate Issuance Service" after 7:00 a.m. on the day following the day you receive an e-mail notifying you of the change of address.
- If you wish to register, change, or delete an address outside of Japan, please access K-Support > Applications and apply from "Registration of Overseas Address (for Student)" or "Registration of Overseas Address (for Guarantor).
Change of Guarantor
How to Apply
Log in to K-Support, go to the left menu > "Applications" (on a smartphone, go to the upper right menu button > "Applications"), access "Change of Guarantor" and apply for the change.
Students' guarantor should be an adult who resides in Japan, is responsible for supporting their family financially, and he or she must be able to bear responsibility for the studentʼs academic fees and expenses and personal aff airs. If possible, please designate one of studentʼs parents. If it is not possible for the studentʼs parent to act as the guarantor, please designate a guardian, such as an older sibling, uncle, aunt, or equivalent.
Required Documents
Prepare the following documents and upload them to the file attachment field of the application form.
Required documents:
- Document of agreement to change guarantor
- The new guarantor's identification document (with address listed) *Please attach one of the following:
- Driver's License (valid and issued in Japan)
- Driving History Certificate (issued on or after April 1, 2012)
- Individual Number Card (valid, only the front side, may be submitted in its original case, no notification cards allowed)
- Health Insurance Card or Confirmation of Eligibility Certificate (redact the insurance numbers)
- Residence Card (valid, both sides)
- Certificate of residence (issued within the last three months, should not include Individual Number)
Notes
If a student is unable to choose a person residing in Japan as a guarantor, the student may assign a person who lives in a foreign country. In such cases, please attach the certificate of residence with your country's official stamp as new guarantor's identification document.
Also, in this case, Please make sure to report an address in Japan as a contact person in case of emergency by going to K-Support > "Application" > "Emergency Contact in Japan".
Change to Family Name or Given Name (Student or Guarantor)
How to Apply
Log in to K-Support, go to the left menu > "Applications" (on a smartphone, go to the upper right menu button > "Applications"), access "Change of Name (for Student)" or "Change of Name (for Guarantor)" and apply for the change.
Required Documents
Prepare the following documents and upload them to the file attachment field of the application form.
Required documents:
- Name change documents *Please attach one of the following:
- Certificate of Individual Records or Extract of the Family Register (issued within the last three months)
- Certificate of Residence with former surname (issued within the last three months, should not include Individual Number)
- Driver's License with former surname (valid and issued in Japan)
- Individual Number Card with former surname (valid, only the front side, may be submitted in its original case, no notification cards allowed)
Notes
1. Reissuing temporary student ID cards :
For students who undergo a name change, a new student ID card reflecting that change will be issued after the procedures have been completed in the system.
After registering the name change in the system, be sure to access K-Support and navigate to "Applications" (申請) and select the "Reissue of Student ID Card" (学生証再交付) option to make your application for a new card.
*There is no processing fee for reissuing applications due to a change in name.
You will need to exchange your old student ID card for the new one, so bring your old card with you.
*If you have lost your old ID card that has your former name, you will be charged a processing fee of 2,000 yen.
*Read the "Notes" section carefully when making your application.
*It typically takes several days from your application for your student ID card to be reissued.
2. Other Notes
- After approval of your application, please inform the faculty in charge of the course you are taking that your name has changed.
