Notification of Off-Campus Student Activity
【Off-campus Research/Academic Activity Report Form】

Until the spring semester of the 2023 academic year, students were required to submit a "Off-campus Research/Academic Activity Report Form" (paper form) when engaging in off-campus activities, but from the fall semester of the 2023 academic year, students will be required to submit an "Notification of Off-Campus Student Activity" (domestic) and "Overseas Activity Application" (overseas) using the Online System for Student Club Activities.

Notes on Submission

The student's submission operation alone does not complete the submission process.
Submission is only complete after receiving approval from the faculty member by the submission deadline (23:59 four business days prior to the first day of the activity).

【Example】 Submission deadline for the first day of activities on 10/2 (Mon.)  ⇒  9/26 (Tue.) 23:59
(*Business days are days excluding Saturdays, Sundays, national holidays, Keio University holidays, and campus holidays. )

After the submission deadline, the submission operation will be disabled. Please be careful for the submission deadline when preparing your application.

Q. 提出が完了できず、「今回の届出は無効となり、当該活動は実施できません」という通知が送られてきた。活動は禁止されてしまうのか。

⇒ 活動自体が禁止されるわけではありませんが、提出期限を守って申請を提出すること原則としています。
  申請を作成してから、提出期限が過ぎた場合には【問い合わせフォーム】にご連絡ください。

Q. 提出期限を過ぎたため、届出を作成できない。活動は禁止されてしまうのか。

⇒ 活動自体が禁止されるわけではありませんが、提出期限を守って申請を提出すること原則としています。
  次の活動からは、期日を守って必ず申請を提出してください。

Q. 申請を作成したが、活動の予定がなくなった。

⇒ 作成した申請を削除してください。

Code of Conduct for Off-Campus Student Activity

When attending an off-campus academic activity, the students must comply with the following rule.

1. When attending an off-campus academic activity, the student shall act responsibly as a member of Keio University.

2. When conducting research at an off-campus institution, the student shall comply with the following rules regardless of whether a joint research agreement is concluded or not, to avoid disputes which may arise between parties.

  • The student shall use the data and results obtained through the research after thorough discussion with the institution about ownership.
  • The student shall not plagiarize or leak to third parties the confidential or personal information obtained through the research.
  • The student shall not remove without permission any items possessed by the off-campus institution including materials, drawings, electronic media, research materials and documents from its office.
  • The student shall adhere to the safety and health regulations stipulated by the off-campus institution and cooperate in taking necessary measures and precautions for health and safety.
  • The student shall adhere to the regulations or policies for the handling of general information and protection of personal information stipulated by the off-campus institution and cooperate in taking necessary measures to protect information leakage.

3. The student shall register the overseas travel insurance beforehand.

4. The student shall inform their guarantor about the off-campus activity in case of emergency.

<Submission Process>
1) Select "Societies, laboratories and offices, etc.".
2) Select [Notification of Off-Campus Student Activity / Overseas Activity Application Form] to create a new notification.
3) After entering, saving, and submitting the required information, a notification will be sent to the faculty member requesting approval.
Once the faculty member's approval is done, the submission is complete.
